Creating investigation records

You create an investigation record to address an unresolved incident or defect. You can record the results of an after-action review, a root cause failure analysis, or a failure mode effect and criticality analysis.

About this task

To control the status of an investigation record, do not relate the investigation record to a global record.

Procedure

  1. In the Investigations application, click New Investigation.
  2. To quickly add data to multiple fields, apply a template and then edit the fields as needed.
  3. Assign an owner or owner group to the investigation.
  4. Specify the primary asset, location, or configuration item that is related to the investigation. You can specify a value in one or more fields. If the investigation affects multiple assets, locations, or configuration items, specify additional information in the Multiple Assets, Locations and CIs window.
  5. If the investigation is a global, regional, or local investigation that is not closely related to an existing issue, select the Global Issue, Regional Issue, or Local Issue checkbox. You can select only one checkbox. To select a different checkbox, first clear the current selected checkbox.
  6. If the investigation is closely related to an existing issue, specify that issue in the Related to Issue ID field.
    The Issue Class field is populated with the class (for example, Incident or Service Request) of the related issue. If the investigation is a global record, the status of the related issue is updated when you update the status of the investigation.
  7. Specify the audit and survey record that is associated with the investigation.
  8. In the associated tabs, you can specify the details about the sequence of events that apply to an incident, including information about witnesses to the events. You can also specify information about activities and scheduling, solutions, and failure report data.
    For example, on the Failure Reporting tab, you can describe the condition of equipment at the time of failure, and associate equipment failures with consequence records.
  9. Save the record.
