  • Supports coordinated care delivery, sharing of care plans, and team-based care.
  • Enables the creation of longitudinal and data-driven care plans.
  • Enables care managers to provide meaningful, personalized, care pathways for individuals that can drive behavior change that lead to better health and reduced costs.
  • Increases care manager productivity, with guided care planning, and follow-up by using integrated tools that can enable care managers to spend more productive time with clients.

Supervisors can access dashboards to see charts with program and demographic statistics for clients, or create their own custom reports and dashboards, as needed.

Notes Highlights uses natural language processing and cognitive computing to identify, categorize, and rank notes highlights. Notes highlights are generated from the last year of unstructured, shared, and published notes and their associated comments, and are updated as new notes or comments are added.
