Managing care teams

You can create and manage a care team to ensure that all the needs of an individual are met.

Care teams
Care teams are care professionals in an organization who are involved in the care of a client. A care team member might or might not fulfill a specific role in a client’s care and has access to the client. Care team members can change over time according to client needs.
Registering care team members for a specific client
You can register care team members for a specific client. You must register care team members who do not have an IBM Watson Care Manager account so that they can be added to care teams. To avoid duplicate registrations, ensure that you search for existing care team members before registration.

Adding care team members individually
You can add an individual care team member who has a required role or skills to a client's care team. If organization units are enabled for your organization, the care team member must belong to your organization unit.
Adding groups of care team members
To save time, instead of adding care team members individually, you can add one or more groups of care team members to a care team, if configured. When you add a care team group, all members of the group are added to the client's care team.
Removing care team members
You can remove care team members from a care team.
Modifying care team members' details
You can update the details of care team members, including their care team roles and membership of the core care team. For registered care team members only, you can add skills, specify their relationship with the individual, and update their demographic information.
Reviewing current and past members of a care team
You can view and maintain the core team members who are currently involved in the care of an individual and determine who has a primary or lead role in a client's care management. You can also see the history of care that is provided by past team members and if needed, you can reinstate those members on the team to keep the client's relationship with the care team they had previously.
Searching for care team members
Search for care team members who either have IBM Watson Care Manager accounts or are registered care team members. Registered care team members do not have access to the IBM Watson Care Manager application.
Assigning a care team member's clients to you or other care team members
Assign a selected care team member's clients (single or bulk selection) to you, another care team member based on their capacity, roles, and skills, and if configured, one or more care team groups (if configured).

Reassign a selected care team member's clients (single or bulk selection) to you or another care team member based on their capacity, roles, and skills.
