Registering care team members for a specific client

You can register care team members for a specific client. You must register care team members who do not have an IBM Watson Care Manager account so that they can be added to care teams. To avoid duplicate registrations, ensure that you search for existing care team members before registration.

Before you begin

Care team members either have IBM Watson Care Manager accounts or are registered care team members. Registered care team members do not have access to the IBM Watson Care Manager application. To register a care team member, you must know at least their name and role.

You search for a care team member to add to the care team by entering some search criteria. Full and partial matches are returned and special characters are supported, for example, you can add a care team member with a last name of O'Neill. However, note that searching for O'Neill will not return O Neill or ONeill in the search results and vice versa.

If your search results do not list the care team member, you can register them in the application.

Procedure

  1. To see your clients, from the Side Navigation Menu, select Clients > My Clients, or search for other clients.
  2. Click a client name.
    Their Summary View opens.
  3. Select Team > Care Team and then click Add.
    The Add Care Team Member wizard opens.
  4. To avoid duplicate registrations, search for the care team member to ensure that they are not already registered. If they are already registered, select the care team member and click Add Selected to add them to the care team.

    If the care team member is not already registered, click Register New.

  5. Enter the care team member details:
    • Title
    • Name (mandatory)
    • Suffixes - Choose one or more suffixes to append to a care team member's name to reflect that person's position, educational degree, accreditation, office, or honor.
    • Gender
    • Roles (Mandatory) - Choose one or more care team roles to add for the care team member.
    • Add as Primary- Select to associate the care team member with the primary or lead role in a client's care management.
    • Add as Core - Select to add the care team member to the core care team. Core care team members are considered key to the delivery of an individual's care and typically interact regularly with the individual.
    • Relationship - Select the relationship between the registered team member and the individual. For example, a caregiver might be a friend or a family member.
    • Skill - Search for and select a skill to add for the care team member.
    • Phone Numbers
    • Email Address
    • Frequency - Choose from listed frequency values that represent the number of visits or meetings the care team member has with an individual.
    • Consent Records - Either select All, or choose one or more listed consent records that represent the personal health information an individual is willing to share with the care team member. You cannot do both. The All option indicates that the client is providing all of the types of consent records that are configured in the application.
    • Distance - Enter the distance between the location of the care team member and individual, in miles or kilometers.

  6. Click Save to register the care team member.
