Monitoring

You can monitor your clients, respond to alerts, and continually update their information and status. You can track your work and update your planned work according to any changes.

Monitoring my alerts
You can see a list of all alerts that you were notified about on the Care Manager Workspace. You can edit, close, or comment on the alerts.
Capturing alerts
You can create, modify, and close alerts, which alert care team members to something that needs action or attention from the care team. Alerts are shown in the context pane, and alert notifications, which display the open alerts that you have been notified of, are shown on the Care Manager Workspace.
Updating clients' priority
Client priority indicates the level of importance of a client's care. For example, High, Medium, Low, or Not Set. You can update a client's priority at any time if their care needs increase or decrease.
Monitoring tasks that apply to clients
If organization units are enabled for your organization, you can view a list of tasks that users of your organization unit created. Tasks can also be automatically generated from a program, assessment, or questionnaire. By default, the Tasks list page shows your open tasks.
Viewing clients with appointments
For clients where you are a member of their care team, you can see which clients have appointments so that you can prepare for upcoming appointments with your care team.
Recording touchpoints
You can record all contacts with a client. A touchpoint is an inbound or outbound contact or attempted contact between the care team and either the client or another individual in relation to a client. You can edit and delete manually added touchpoints. You can delete touchpoints that come from integrated systems, but they cannot be edited.
Recording utilization
Utilization is the measure of a client's use of the clinical services that are available to them. You can record a client's use of clinical services, such as hospital encounters or specialist visits. Your organization can then use this information to understand the usage and outcome patterns for services to drive improvements. You can add, edit, and delete utilization records, and update a utilization record with notes and comments.
