APAR Error Description / Circumvention

When an error is logged by the IOPless optical driver during IPL
from that device, the IPL will fail with little or no
information about the actual failure.

CORRECTION FOR APAR MA45412 :
-----------------------------
Apply this fix before the LIC code is saved, if an error occurs
when that code is used for IPL, a C60041F5 SRC will be logged.
Word3 will include the URC requested.  Additional words may be
included with machine internal data.
