Problem conclusion

• a number of the X212 rules that  check that TRN/STC segments
  only present at one level  had incorrect scope in the
  hipaa_x12_qualifier.dat and hipaa_x12_qualifier.json files, so
  errors reported on valid data.  Corrected scope to the HL loop
  for these rules.
