IT41408: Invalid queue manager name and PUTAPPLNAME in channel statisticsmessages

Error description

• When viewing a channel statistics message, both the PUTAPPLNAME
  of the MQMD, and the queue manager name within the statistics
  message are incorrect.

  PROBLEM DESCRIPTION:
  A logic error meant that an internal variable holding queue
  manager name was not initialized correctly when creating the
  channel statistics message, resulting in both the PUTAPPLNAME
  and queue manager name of the message being populated with the
  incorrect value, instead of the queue manager name as expected.
  

Problem conclusion

• The IBM MQ queue manager channel statistics logic has been
  updated to correctly insert the queue manager name into the
  PUTAPPLNAME and queue manager name within the statistics
  message.
