IT39666: Triggered channel does not start

Error description

• A triggered channel might not start, when an instance of that
  channel is being ended normally at the same time.

  All users of MQ message channels (client channels are not
  affected by this issue).

  PROBLEM DESCRIPTION:
  Unlikely timing windows were detected in the MQ queue manager's
  channel start logic which caused the presence of race conditions
  where the channels start/stop code occasionally might not start
  a channel when needed - for example, when the channel is
  triggered to start by a message arriving on the transmission
  queue, at about the same time the channel is ending normally.
  

Problem conclusion

• The race conditions in the channels start/stop code have been
  corrected.
