Referrals data

The Referrals folder groups data relating to client referrals. Use the data sets in the Referrals folder to create referral reports. The data model shows how the data sets relate to each other.

For example, you can create the following reports:

A list of client referrals by status and referral type for a specific time period.
The average number of days that referrals are assigned to a care manager before they are disposed.
The number of referred clients who are not assigned or enrolled in a program.
The number of referrals that are not yet assigned to a care manager.
The number of referred clients by their cohort and referral reason.
A list of deactivated clients for a specific time period, the care manager who deactivated each client, and the reason that the client was deactivated.

You can filter your report content by applying filters to the data items in the reports. For example, you can filter referral reports by Referral Status, Care Manager, Program Name, and Program Status, if needed.
