Planning actions
Client actions are care plan activities that clients can complete to help them to achieve a goal. Client actions must be associated with one or more goals. You can view and manage client actions on the care plan and associate them with the appropriate goals.
About this task
Actions can be configured to be automatically associated with a goal as a result of completing an assessment or questionnaire on a program.
Actions can be configured to be added to the plan as completed when a care team member completes the action by completing the assessment or questionnaire. For example, a completed action of Call client to discuss care plan can be added when a care team member completes an outreach questionnaire.
