Running a questionnaire

You can run a questionnaire to gather information about a client.

About this task

If configured to be shown, you can select the source of the questionnaire (mandatory) and the name of the original source system where the questionnaire originated (optional).

Procedure

  1. Sign in to your account.
    Your home page opens.
  2. To see your clients, from the Side Navigation Menu, select Clients > My Clients, or search for other clients.
  3. Click a client name.
    Their Summary View opens.
  4. Access the Program page by using one of the following options.
    • In the Programs card on the Summary page, click the name of the program.
    • On the Programs page, click the name of the program.
    The Program page is displayed, including a list of all modules for the program.
  5. Identify the modules that you want to work with.
    Expand modules to find questionnaires.
  6. Click the questionnaire that you want to run.
    The questions of the questionnaire are displayed. Color might be configured to let you emphasize the importance of certain questions or information with clients. Hyperlinks to external websites might be configured to let you direct clients to supplementary reference information.
    • The first time that you run the questionnaire, it is blank and ready to run.
    • If the questionnaire was run previously, previous answers are displayed for your reference. Click Repeat to start a new instance of a completed questionnaire.
  7. From the Source menu, if applicable, select one of the configured values that indicate the source of the questionnaire.
  8. From the Original Source System menu, if applicable, select one of the configured values that indicate where the questionnaire originated.
  9. Enter the client's answers to the questions. If you are entering a multiple-line answer type, the character limit is 4,000.
  10. Click Save.
    The answers are saved on the questionnaire, and you can return later to complete it.
  11. Click Complete to finalize the questionnaire.
