VIOS viosupgrade error "ioslevel file is not present in provided VIOS image. Please Check if the image is valid"

Question

I am trying to upgrade VIOS to version 3.1 using VIOS command, viosupgrade, to an alternate disk, but it fails as follows:
$ viosupgrade -l -i /PATH/MKSYSB_FILE -a hdisk#
ioslevel file is not present in provided VIOS image. Please Check if the image is valid.

Cause

The specified mksysb image is not valid.

Answer

Ensure the mksysb image is built correctly as described at How to build the mksysb file for VIOS 3.1 Migration / Installation?
