"Standard Error ... Object reference not set to an instance of an object" when click on 'Open form...', triggered by first opening a separate XLSX file

Troubleshooting

Problem

User closes all programs (nothing else running in the background). User double-clicks on an XLSX file (to open it in Excel). - User clicks on the 'Controller' add-in menu - User clicks on the 'Open Form' icon An error appears.
