A new column heading has been added to the index file read by
post-processor exit programs.  The sample post-processor exit
programs need to be updated to handle the column heading
correctly.

The QRLMSFAX sample program incorrectly handles spooled files
that have file numbers greater than 9999.

file numbers up to 999999.
