'This operation has been canceled due to restrictions in effect on this computer' when running Standard Report

Troubleshooting

Problem

User clicks on 'maintain - account structure' report. User selects 'codes' and a form, and clicks 'preview'. Browser window opens as expected, and the report appears correctly in HTML format. User changes selection from 'HTML' to 'XLS'. A popup 'File Download' message appears, with the options 'OPEN', 'SAVE' and 'CANCEL'. User chooses OPEN. The Internet Explorer session now changes its view so that it appears to run Excel (with its cells etc.) embedded inside the IE browser screen. * However, it has a MIX (from both Excel & IE) of options inside the header (see screenshot below for an explanation).

NOTE: The above is different from the 'expected' formatting, where there is no header row (i.e. there is no 'file / edit / insert / format / tools / data / controller /window / help' menu row).
