"Not Authorized" when logging in to Controller Web

Troubleshooting


Problem

When a user logs in to Controller web, you see a "Not Authorized" error message.
You see the following the fcm web logs - 
[org.apache.axis.utils.JavaUtils] Unable to find required classes (javax.activation.DataHandler and javax.mail.internet.MimeMultipart). Attachment support is disabled.  [com.ibm.cognos.fcm.web.ui.infrastructure.auth.cam.CamAuthenticationProvider] Authentication rejected: '' (org.springframework.security.authentication.BadCredentialsException: Cannot validate passport: ; nested exception is: java.net.ConnectException: Connection refused: connect) 
[com.ibm.cognos.fcm.web.ui.infrastructure.auth.cam.CamAuthenticationProvider] Authentication rejected: '' (org.springframework.security.authentication.AuthenticationServiceException: Invalid authentication credentials.)
