Symptom
QLogic 4 Gigabit (Gb) Single-Port or Dual-Port Host Bus Adapter (HBA) Fibre Channel cards cannot be configured under Unified Extensible Firmware Interface (UEFI):
System Settings -> Storage setup menuThe screen just shows a blue box for several seconds, then returns to the previous UEFI menu.
Affected configurations
The system is configured with one or more of the following IBM options:
- QLogic 4 Gb PCI-X Fibre Channel Dual-Port HBA for IBM System x, option part number 39M5895, any replacement part number
- QLogic 4 Gb PCI-X Fibre Channel Single-Port HBA for IBM System x, option part number 39M5894, any replacement part number
- QLogic 4 Gb PCIe Fibre Channel Dual-Port HBA for IBM System x, option part number 39R6527, any replacement part number
- QLogic 4 Gb PCIe Fibre Channel Single-Port HBA for IBM System x, option part number 39R6525, any replacement part number

Workaround
Select the System Settings -> Devices and UEFI and find QLogic card information. Enter the command line configuration page.
Additional information
When Unified Change Management (UCM) support was enabled, it did not include support for 4 Gb QLogic adapter options, because the 4 Gb adapter options were already at the end of product life in the year 2013.
