Problem
"I/O Module Fault" is reported by the Advanced Management Module (AMM) for a IBM BladeCenter SAS Connectivity Module, Option part number 39Y9195, running in a BladeCenter H chassis. The user is unable to power cycle or reset the SAS Connectivity Modulethrough the AMM, but the SAS Connectivity Module otherwise remains functional.

Symptom
"I/O Module Fault" is reported by the Advanced Management Module (AMM) for a IBM BladeCenter SAS Connectivity Module, Option part number 39Y9195, running in a BladeCenter H chassis.
The user is unable to power cycle or reset the SAS Connectivity Module through the AMM, but the SAS Connectivity Module otherwise remains functional.
Affected configurations
The system may be any of the following IBM servers:
- BladeCenter H, type 7989, any model
- BladeCenter H, type 8852, any model
The system is configured with one or more of the following IBM Options:
- IBM BladeCenter SAS Connectivity Module, Option part number 39Y9195, replacement part number 39Y9193
This tip is not software specific.
Solution
This rare behavior has been corrected in First Quarter 2012 AMM firmware code, Version 3.62O, Build ID: BPET62O.
The file is available by selecting the appropriate Product group, type of System, Product name, Product machine type, and operating system on IBM Support's Fix Central web page, at the following URL:
Workaround
Restart the primary AMM or fail over to a secondary AMM, if available, to re-establish communication between the AMM and the SAS Connectivity Module.

IBM is investigating this issue. Possible root cause has been identified.
