Solution
This issue is corrected by IBM SAS hard drive update program
version 1.04.
IBM strongly recommends applying this firmware to prevent
undetected data corruption.
As always IBM recommends backing up all data before applying
hard drive firmware updates.

Workaround
Ensure hard drive write cache is disabled.
This setting can be changed to user preference once the new
firmware is applied.
If ServeRAID Manager or MegaRAID Storage Manager is installed,
the user can view and change the hard drive write cache state from
within this utility.
Otherwise, the BIOS of the controller (which the drives are
attached to) can be used to disable the drive write cache.
For LSI controllers, the cache state can be changed in the
MegaRAID Storage Manager utility, or the WebBIOS utility.
For Adaptec controllers the cache state can be changed in the
ServeRAID Manager utility or using the CTRL-A key combination
during Power On Self Test (POST) to enter the adapter's BIOS
settings. By default the Adaptec controller has SATA drive write
cache enabled.
Additional information
SAS hard disk drives have drive write cache is disabled by
default.
SATA hard disk drives have the write cache enabled by
default.
Please verify the drive write cache state for SAS or SATA hard
disk drives using the ServeRAID Manager, MegaRAID Storage Manager,
or adapter BIOS utilities. Disabling the hard disk drive write
cache will exempt you from experiencing this issue.
Users are strongly encouraged to implement the workaround or the
apply the firmware update.
