Optical Pass-Thru Module may not power up on insertion - IBM BladeCenters

Resolving The Problem

Symptom
The Optical Pass-thru Module (OPM) BladeCenter switch module may not power up on insertion. Approximately 1 in 50 switch module insertions will not result in the switch module automatically powering up.

Affected configurations
The system may be any of the following IBM BladeCenters:
  • IBM BladeCenter, type 8677, any model
  • IBM BladeCenter HS40, type 8839, any model
  • IBM BladeCenter HS20, type 8678, any model
  • IBM BladeCenter HS20, type 8832, any model
  • IBM BladeCenter JS20, type 8842, any model
  • IBM BladeCenter T, type 8730, any model
  • IBM BladeCenter T, type 8720, any model

The system is configured with the following option:

  • IBM BladeCenter Optical Pass-thru Module Option part number 02R9080, replacement part number (FRU) 02R9081.
Solution
None. This is a system limitation.

Workaround
Recovery is possible either by reinserting the OPM switch module or by cycling the OPM power, on the chassis.

Additional information
This behavior is the result of a rare event during a hardware hot-plug insertion and is cleared by reinsertion.
