USB test error codes

If any of the following USB errors occur on devices not directly attached to a USB port on the xSeries 450 (for example, a USB mouse attached to a USB keyboard with an internal USB hub), connect the suspect device directly to the xSeries 450 and re-run the failing test to isolate whether the failure is with an intermediate device, the device being tested, or the USB subsystem of the xSeries 450. In cases where the direct attachment passes, replace the intermediate devices and re-test in the original configuration before replacing the device being tested or the I/O board.
