Code modified to prevent the IO failure.
Problem summary
Possible I/O failure during storage controller upgrade case. In such scenario, the dynamic tracking logic of AIX Fibre channel stack could not get recovered and leading to I/O failure.
Problem conclusion
Modified AIX FC drivers's dynamic tracking logic to handle error condition effectively to avoid I/O failure
