Problem conclusion

• If subelement procedure code (SV10102 in 837P, SV20202 in 837I,
  SV30102) was not present, we generated an SVC in the 277CA with
  XXXXXXX so 277CA was valid. We have corrected the compliance
  check and hipaa_data_compliance maps to also generate this if
  the input subelements are all pad characters.

  Healthcare Pack HIPAA 10.1.0, 10.2.0, 10.2.1, 10.2.2.
