Description
When performing DLPAR operations, there is a possibility that the adapter is not properly removed from the system, which causes future DLPAR operations to fail. Typical symptoms include error messages from the Hardware Management Console (HMC) similar to the following example:
DLPAR command failed
Error Details STDOUT:HSCL2929 The dynamic removal of I/O resources failed: The I/O slot dynamic partitioning operation failed. Here are the I/O slot IDs that failed and the reasons for failure:

Validating PHB DLPAR capability...yes.
Isolation failed for 20000143 with -9001
Valid outstanding translations exist.

Workaround
There is no workaround for this issue at this time. Instead of using the DLPAR operation, you can shutdown the logical partition before you remove or add Qlogic FC adapters to the configuration.

The resolution to this issue is related to Power10 system firmware. Once a fix is verified in a future firmware build, it will be included in a future update. If the issue is still seen after the fix, please reach out to IBM support for further assistance.
