Error description

• The servers that are shown in Storage Insights are created
  based on the host mappings that are identified on the storage
  systems when the storage system is probed.
  
  Storage Insights should automatically create, update, and
  removes servers as appropriate based on the changes to the
  host mappings.
  
  This includes when a server host name changes.
  Storage Insights is not properly handling this when a host
  name has changed, resulting in duplicate servers listed in
  the GUI.
