Problem summary

  • When flashing a FW image that prevents the device from becoming
operation again after the PCI reset can result in a crash.
Crash will only occur when doing the FW update and not while
configuring the adapter normally after reboot.

Problem conclusion

  • Add check during the resume error handling to see if a FW
update had just occurred and perform a restart instead of an
assert to try and recover from the failure. If recovery is
impossible after the restart, then put the device in a DEAD
    state.
