There is content available for IBM® Watson™ Care Manager that is developed by the IBM Watson Health clinical informatics team and by third-party organizations. Customers who would like to request content from the content catalog can do so by opening a case on the Watson Health Support Portal.


Introduction

IBM Watson Care Manager is a cloud-based care management solution to help your organization to focus on individual-centered care. Care teams can capture and assess structured and unstructured information, select targeted programs, and create individualized care plans. Care plans can be adjusted to address changing biological, psychological, social, and functional needs.

So that IBM Watson Care Manager can support the varying workflows that different organizations might require, administrators can configure content for their care teams, for example programs, assessments, questionnaires, goals and actions. To speed up the configuration of Watson Care Management for your organization, there is content available that is developed by the IBM Watson Health clinical informatics team and by third-party organizations that you can request for your implementation of Watson Care Manager.

Opioid Risk Tool (for Men)

The Opioid Risk Tool (ORT) is a brief screening tool to predict which individuals may develop aberrant behaviors when prescribed opioids for chronic pain. The Opioid Risk Tool is scored on a scale of 0-26. A score of 4 or more is considered to be a moderate risk. Generally, the higher the ORT score, the more likely it is that the individual may develop aberrant behaviors if using opioids.

Health Leads Social Needs Screening Toolkit gathers information about social determinants of health (SDOH) which are factors that influence health and well-being including behavior, access to health services, and the environments in which people live and work. Organizations assess SDOH in order to improve the lives of their patients through improved access to appropriate health care while relying upon evidence-based interventions and strong partnerships with the community, members of the health care team, social service agencies, the patient and their caregivers.
