Run-time error '429': When launching Contributor Administration Console

Symptom

When trying to open the CAC:

"An error has occurred starting Cognos Planning - Contributor Administration. Main form failed to initialize."

After pressing OK to the above error message, another error message appears:

"Run-time error '429':
