"Unable to authenticate" error or blank screen after authenticating with the OIDC provider when accessing Cognos Connection

Troubleshooting


Problem

Authentication fails when accessing Cognos Connection. 
Using an OIDC namespace.
The namespace tests successfully in Cognos configuration.  But fails to authenticate on the website. 
The following document was used to set up the namespace.

Symptom

When accessing Cognos Analytics and logging in with the same Credentials nothing happens or the following message is issued: "Unable to Authenticate".
If anonymous access is enabled the user does not change from Anonymous.
