  This problem occurs when servers are added to the cluster@port
  combination AFTER rules have been added for the cluster@port.
  

Problem conclusion

• Rules and servers are stored based upon the cluster@port
  combination. The manager attempts to obtain the servers from
  the cluster@port to compute the weights but expects the
  servers to be ordered before the rules.  Code corrected to
  remove this assumption.
  
  Fix applies to all operating systems. Fix can be avoided by
  adding servers to the cluster@port before adding rules for the
  cluster@port.
