All users get "An error has occurred in the script ...'CCognosViewer' is undefined" when running Standard Reports, caused by application pool failure

Troubleshooting


Problem

User runs a standard report (also known as a 'system report'). An error appears.All users affected.
 
System may have been working successfully for a long time (several years) before the problem suddenly occurs.

Symptom

An error has occurred in the script on this page

In addition, if the user launches Cognos Connection (http://servername/ibmcognos) then they get this error:

Service Unavailable

HTTP Error 503. The service is unavailable.
