Error description

• Steps to reproduce.
  1. Create a Form which has a Radio button field as following in
  a database.
  
  Field Info tab
  Type: Radion button, Editable
  Control tab
  Choices: Enter choices (one per line)
  aaa|0
  bbb|1
  Default value: "aaa"
  
  2. Create a new document using step 1 form via Notes client,
  then "aaa" is checked as default.
  3. Create a new document using step 1 form via Web browser.
  
  Problem:
  There is no check on radio button in a new document.

• Set default value as alias (ex. "0") or do not use alias.
