Error description
If the disk device has a block size that is not 512 bytes, such as 4KB block size, then there is a possible exposure where a crash could occurr. If the disk device fails when a non-block aligned read or write I/O is pending it could result in a crash in scsidisk_fail_disk() function.

Problem conclusion
Properly handle failing of pending non-block aligned I/O when the entire disk is failed.
