Error description
When a higher priority path recovers after failure the IO does not fail back to it. errpt shows path recovery ok.
Local fix
Change the path priority to force them to be re-evaluated. e.g. if vscsi0 has priority = 1 and vscsi1 has priority = 2, and vscsi0 is not getting selected: # chpath -l hdisk0 -p vscsi1 -a priority=1 # chpath -l hdisk0 -p vscsi1 -a priority=2
Problem summary

Problem conclusion
When path priorites are reevaluated for all the paths on a path coming back online, account for both fail over and round robin cases.
