Error description
If an inittab process has been switched from 'respawn' mode to another mode like 'once' with the chitab command, init will write an entry to wtmp once per minute regarding this process. If auditing is configured a certain way, turning on auditing may cause these writes to happen much quicker (multiple times per second). Likewise, any other activity that might send SIGCHLD to the init daemon would cause the writes to happen more frequently.

Problem summary
If an inittab process has been switched from 'respawn' to 'once' and the process is killed, init writes entry of this process into wtmp file once per every minute thereby flooding the file with so many entries
Problem conclusion
Needs to clean the process table entry for this process whose mode is chnaged from respawn to other mode after trhe process is killed. i.e remove the dead process entry which prevents the writing of the init into wtmp file
