Error Message: "ORA-01654: unable to extend index" Engine not responding

Troubleshooting

Problem

Unable to log into any IBM Initiate application. The following errors are displayed in the engine logs: [2246] ERROR SQLExecute: oexec(1:2) failed, ORA-01654: unable to extend index EMPI.MPI_AUDHEAD0 by 8192 in tablespace EMPI_DATA. [2246] ERROR ODBC [01000] ORA-01654: unable to extend index EMPI.MPI_AUDHEAD0 by 8192 in tablespace EMPI_DATA (DBError=1654) [2246] ERROR MAD_PkgExecStmt: SQLExecute(mpi_audhead) failed.
