"ControllerForms12 ... An item with the same key has already been added" error when opening specific Data Entry form

Troubleshooting


Problem

User launches main Controller client ('classic' client). User clicks "Company - Data Entry - Reported Values". User chooses parameters, including the form name.
User clicks 'Open'. An error appears.
Problem only affects one/some form(s).
