'Model initialization failed. Cause already initialized' error when accessing Rule Execution Server console

Problem

When the Rule Execution Server console is accessed, a 'Model initialization failed' exception is seen.

Symptom

The following exception stacktrace is seen when accessing the Rule Execution Server console:
