"Microsoft Excel has stopped working" error when launching Excel (if Controller add-in is enabled)

Problem

User launches Excel. User receives error message. If user tries to re-launch Excel afterwards, then Excel warns that there was a a problem with the Controller add-in, and it suggests to disable it.

Symptom

Initial error:

    Microsoft Excel has stopped working

If user tries to re-launch Excel afterwards, then they may see this message:



Excel experienced a serious problem with the 'C:\program files\ibm\ibm cognos controller link for microsoft excel\asxloader.controller.excellink.dll' add-in. If you have seen this message muiltiple times, you should disable this add-in and check to see if an update is available. Do you want to disable this add-in?
