"Invalid index. (Exception from HRESULT: 0x8002000B (DISP_E_BADINDEX))" when trying to open Data Entry form, caused by incompatible Excel plugin

Problem

User clicks "Company - Data Entry - Reported Values". An error appears. A similar error occurs when trying to click the green 'X' (Controller link button) or when trying to run a report.
