Intermittent crashes ("System.AccessViolationException") opening Data Entry form when using unpatched Microsoft Office 2016 click-to-run

Troubleshooting


Problem

User launches the 'main' Controller client. User clicks 'Company - Data Entry - Reported Values'. User chooses parameters, and successfully opens a data entry form (in Excel). User then tries to perform a similar task (to open a different data entry form). Excel crashes with an error. The problem is intermittent.

Symptom
