How to create connection to Microsoft SQL Server with Windows Authentication

Question

Microsoft SQL Server connection allows access to SQL Server with SQL Server Authentication mode.

However, the connection failed when we use Windows Authentication to access database on Microsoft SQL Server with "Unable to authenticate" error messages.
