'forms could not be opened' error message is shown when viewing a form document after upgrading

Troubleshooting


Problem

When you open a page that contains a form widget that renders a form document, an error message is displayed.

Symptom

When you open a page with a forms widget that renders a form template or document, you see an error message that contains the following:
The form could not be opened due to an internal error.
Closing the error, you still see the form document or template rendered correctly.
