Problem
vhost is in Defined state and attempt to make it Available using chdev command fails
Symptom
vhost shows in Defined state
$ lsdev -type adapter
vhost5 Defined U9009.22G.78F4XXX-V1-C9 Virtual SCSI Server AdapterAttempt to make it Available fails:

Diagnosing The Problem
The adapter ID no longer exists on the VIOS partition's profile. This can happen when the adapter ID is created dynamically without saving the change to the VIOS partition profile. The next time the partition is reactivated, the change is lost.
In this example, the physical location for the vhost in question is "
U9009.22G.78F4XXX-V1-C9"where - 78F4XXX is the Power System serial number
- V1 is the VIOS LPAR ID 1
- C9 is the VSCSI Server Adapter ID, also known as the slot number.
Resolving The Problem
Login to the HMC GUI and verify the VSCSI Server Adapter ID exits on the VIOS partition's profile.
If it does not exist, recreate it dynamically, and save the change to the partition profile. Then, retry cfgdev.
