"Cannot add account ... Cannot retrieve discovery document" when click 'Activate' in Citrix Storefront

Problem

User launches Controller-on-Cloud web portal (Citrix Storefront). User logs into website. User clicks 'down arrow' next to their name, and then clicks 'Activate...' button. Several messages appear.
