• IMS PA incorrectly reports Cancelled transactions in form-based
  reports as completion level=4 and without a start time.

Problem conclusion

• 1. Cancelled transaction remain as completion level=0, Start
  time assigned.
  2. Add a failure code of "REJECT" for Rejected transactions,
  currently blank.
