Error description
When a custom header (bmheader.htm) is configured, all functional icons are missing from the toolbar when using the tradtional framed interface for displaying Book pages.
Local fix
Remove custom header from your configuration. The custom header can be unconfigured by removing (or moving) the HFS (or zFS) file named bmheader.htm. Assuming that a LOGDIR= keyword is configured in your bookmgr.80 file, you should find bmheader.htm in that LOGDIR directory. If LOGDIR is not configured, you should find bmheader.htm in your cgi-bin directory (the same directory in which the EPHBOOKS executable resides).
