PM17931: TRANSACTION SUBSTITUTION EXIT TO SUPPORT FAST PATH TRANSACTIONS AND ADDRESS A FEW OTHER ISSUES

Error description

• Add support for X'5901' IFP Input into Transaction Substitution
  exit and hence subsequent reporting of IFP Trancode in Fast Path
  reports and Form-based reports.
  
  Additional items fixed:
  1) Correct IPIERA30 sample code (fix typo)
  2) Transit Log report incorrectly suppresses originating Lterm
     for non-pgm switches. Correction to not suppress Originating
     Lterm where Transaction has the same Originating Token but
     is not a Program Switch
  3) Sundry dialog fixes (typos)
  4) Loop in IPI4QMGR in ATF reporting.  It occurs at three
     instructions at x'39A' x'3A2' x'396'.  It is due to
     Transactions being "orphaned" when the ATF Correlation and
     Creation token are not consistent.
