PM16412: ACCOUNT IS LOCKED OUT AFTER 2 FAILED ATTEMPTS DESPITE THE PWDMAXFAILURE SET TO 3 WITHIN TIVOLI DIRECTORY SERVER

Error description

• With the pwdMaxFailure set to 3 within Tivoli Directory Server,
  a user will get locked out after only two failed attempts.
  ?Cognos appears to be attempting to login to the directory
  server twice for every 1 time that a user clicks login. This is
  causing user accounts to get locked out prior to hitting the
  actual pwdMaxFailure.??Please see test case for additional
